Open Records Requests

The purpose of the Office of Open Records is to enforce the state’s Right-to-Know law and to serve as a resource for residents, public officials, and members of the media in obtaining public records of the Perkiomen Valley School District. 

Records request forms may be obtained in the District Office, 3 Iron Bridge Drive, Collegeville, PA, or may be printed by clicking the link below. Records requests should be submitted to:
